The scroll in mind is called "Settlement Name Change," and you are correct it is a max quantity of one per use. Once purchased, head to the Domain & Settlement Selection portal (the appropriate server selected), and a window prompt will be open for you to rename the settlement. After that it is a matter of time for the name approval to go through, once accepted (if approved) then you will receive a private message here on the website about it. :)
Settlement Name Change:

Go to the map and click the paper with a pen in the left hand corner. Does it allow you to rename?
Some people were able to rename from the default when they left it as the default name. So it's possible you have a rename currently on it.

Nope it doesn't show up at all on my screen on domain. The legend does, but not a paper/pen.
I would buy the Settlement Name Change if needed, but its not allowing me to do so because the store thinks i own one already, even though I do not - no option in Domains nor under Inventory.

Hmm. Yea thats a weird one, I'd suggest emailing support
(I checked to see if its the surname thing, but it's not cause I have surnames like 3 and it hasn't changed to being max quantity for me)
did your default name selection get approved? It still goes through a manual acceptance by the devs so you might be able to reach out to them and get it halted before it gets approved.

Are you sure? It took a couple of days before I received a notification on my name acceptance and I kept the default settlement name.

*Edit: I noticed its listed in the store under Misc., but it says "Maximum Quantity Reached." I don't have one, but could this be because I have a surname scroll? Or is it just currently locked?
Trying to find this out ^ Name is already locked - trying to buy name change, but can't.
