Some general things you might want to check in addition to putting in a ticket if needed would be ensuring you're logged in when trying to reserve your surname, and giving it some time to see if you hit the button to try to reserve, it is pending, and you've not hit the button again so it thinks you're trying to reserve another name.
I sent an email. There isn't even an option for me to submit for a name. It prompts me to purchase the Surname & Coat of Arms, as I said. I'm logged in.
